Can I change the look and display content of the store?
While you can completely change the entire layout the store lives within and move the store around to different locations on the page, or even to different pages, the contents of the actual store widget cannot be changed, at this time. You do have control, however, and can change the images you’re using, change the prices, etc. You also have a good deal of control within the product description, and can make dramatic changes within that text box. Otherwise, no. 
What do I need to do to create a store? 
In short, add one (only grandfathered sites are allowed to add store instances)! The store is a very easy thing to add to your website. Once you’ve built a website, log into the website as the administrator. Then, you would navigate to a page with a fairly wide spot in the center of the page, click Add Widget then place the widget on the page. Once you’ve added this blob to your page, you will need to establish a connection with your Payment Providers through the eCommerce Tools on the Dashboard, and then establish at least one shipper/shipping rule and method, and then add a product. Once you’ve done this, you’re selling things!

Do I have to have a PayPal account?
Yes, you need to have at least one of the two. You are not required to have both, but it’s simple to do and has the potential
to increase the number of customers
